If you've ever been in a motor vehicle accident (MVA) in Ontario, you may have heard the term "accident benefits", the coverage that pays for your medical care, physiotherapy, and lost income when a collision leaves you unable to work or fully function.
On July 1, 2026, the rules around those benefits changed in a big way. And whether you've been through an accident before or not, this affects every driver in Ontario.
Here's what's happened, in plain language.
First: What Are Accident Benefits, Exactly?
In Ontario, every auto insurance policy must include Statutory Accident Benefits (SABS). No matter who caused the crash, SABS means you can access medical treatment and financial support.
Think of it as a safety net built into your car insurance. It covers things like:
- Physiotherapy, chiropractic care, and massage therapy
- Psychological counselling
- Income if you can't work due to your injuries
- Help around the house or with caregiving if you're unable to manage
Until now, most of these benefits were automatically included in your policy. That's about to change.
The Big Shift: Automatic to Optional
Under Ontario Regulation 383/24, the provincial government is restructuring accident benefits into a two-tier system:
What Stays, No Matter What
These benefits remain mandatory in every Ontario auto policy:
Medical and Rehabilitation Benefits
Attendant Care Benefits
What This Means for You
If you're injured in an MVA after July 1, 2026, your access to rehabilitation care is still protected.
What's Becoming Optional And Why That Matters
The following benefits, which were previously automatic, will now need to be actively added to your policy:
| Benefit | What It Covers |
|---|---|
| Income Replacement | Up to $400/week if you can't work (previously automatic) |
| Caregiver Benefits | Financial support if injury stops you from caring for a dependent |
| Housekeeping & Home Maintenance | Help with tasks you can no longer manage |
| Non-Earner Benefits | For students, retirees, or those between jobs |
| Death & Funeral Benefits | Support for families after a fatal accident |
If you don't add these before an accident happens, they won't be available when you need them. There's no backdating, and no exceptions.
One More Important Change: Who Pays First
Previously, if you had extended health benefits through work, your auto insurer would make you use those up before they stepped in to cover MVA-related treatment.
As of July 1, 2026, this has flipped: auto insurance becomes the first payer for your rehabilitation expenses. Your workplace benefits are now preserved for your everyday health needs, not used up by an accident someone else may have caused.
For MVA patients, this is actually a meaningful improvement in how smoothly treatment gets funded.
Real-World Scenarios: Who's Most at Risk

You're a worker with no disability coverage through your employer.
Without income replacement benefits in your auto policy, a serious accident could mean months without income and no financial backup.
You're a caregiver for a child or family member.
If your injuries stop you from providing that care, there's no support unless you've opted into caregiver benefits.
You're covered under your employer's fleet insurance.
Fleet policies typically don't include optional benefits. You may have no say in what's included, and no income protection if you're injured on the job.
A Silver Lining Worth Knowing
Previously, caregiver and housekeeping benefits required a catastrophic injury designation, a very high legal bar. Under the new rules, a standard impairment qualifies. More people will be eligible for these benefits than before, as long as they've opted in.

What to Do Before or as of July 1, 2026
1. Review your current policy.
2. Call your insurance broker.
3. Opt in. The cost is lower than you think.
4. If you've been injured, don't wait to start treatment.
The Bottom Line
Ontario's July 1, 2026 SABS reform is the biggest restructuring of auto accident benefits in over 30 years. Your rehabilitation coverage, physiotherapy, chiropractic care, and the treatments that help you recover stay mandatory and protected.
But financial benefits like income replacement are no longer a given. For many Ontarians, the difference between having them and not comes down to one conversation with your broker.
Don't leave it to chance. Review your policy, talk to your broker, and make sure your family is covered.
